Online Sports Wagering Checklist: To ensure all documents are received please follow the OSW Checklist. 

Online Sports Wagering Operator Permit Application: To be completed by a gaming entity that offers online sports wagering through computers, digital platforms or mobile applications in not less than three (3) jurisdictions in the United States pursuant to a state regulatory structure.

Online Sports Wagering Vendor Permit Application: To be completed by a company that provides services to a sports wagering operator that the sports wagering operator uses to accept online sports wagers, including geolocation services, know your customer services, payment processors, server host providers, integrity monitoring services, cyber security services and data providers.

Key Personnel*:   To be completed by any person employed in an executive or supervisory capacity by a license holder, permit holder or applicant and who is authorized to make discretionary decisions that exhibit influence or control over gaming operations.

Multi Jurisdiction Application: To be completed by and in addition to a Key Personnel Application. 

*Key personnel must also complete and submit a Multi Jurisdiction Personal History Disclosure Form along with key personell application.
