Incident Report: Use this form to report accidents/incidents. The report should be completed no more than 24 hours after the event. Submit completed forms Online. To be completed by OTB Manager or other authorized individual.

Jackpot Notification: Use this form to report ALL jackpots in excess of $10,000.00. Jackpots exceeding $100,000.00 will trigger verification by the commission and may require additional information. When feasable please report all jackpots $100,000.00 or more at time of payout.

Simulcast Request: WGC approval of a simulcast request is subject to this form being completed by the requesting PERMITTEE and received by the WGC in a timely manner. Requests not received at least six (6) working days, prior to the first simulcast contained in the request may not receive approval in time for the simulcast.

HHR Catalogs: This catalog contains all approved HHR software.  Once a software has been listed as approved in the catalog it is available for any licensed Simulcast/HHR permittee to use at approved locations.
